To protect physician privacy, the Medical Clinic Registry is separate from the Physician Registry. To update physician practice information, physicians must log in to their CPSA Portal.

To determine whether your facility is eligible for registration, please provide information about yourself and the clinic you wish to register. If your clinic meets CPSA’s registration criteria, we'll provide you with next steps after you submit this form.
